Squire For Hire Complete the tutorial. The tutorial is available when you first launch the game, or from the New Game menu if you skipped it the first time.
The Call To Adventure Complete The Prologue. Win the underground fighting ring in the stables and proceed to the first proper Chapter of the game.
The Road Of Trials Complete Chapter 1. Defeat The Monk.
The Magic Flight Complete Chapter 2. Defeat The Baroness.
Master Of Two Worlds Complete Chapter 3. Defeat the demons and close the portal.
No True Ending Complete Chapter 4. Defeat The King.
Sidewinder Complete a side quest. Side quests can be skipped to recover health, so be sure not to do this if you want the Achievement!
Taskmaster Complete 25 side quests. They don't all have to be on the same playthrough.
Human Resources Hire a party member. Party members can be hired at Taverns. If you decline to hire anyone in Story Mode, you'll sometimes get events where you can hire a party member for free later on.
Going Equipped Equip an item purchased from a Blacksmith Equipping an item received as a reward or from dialogue doesn't count; it has to be one that you purchased.
Juiced Start a fight with three Consumables equipped at once. Each party member can have one Consumable equipped, so you need a full team for this.
Necromancing The Stone Resurrect a party member at a Necromancer. Resurrections cost 50 Gold.
Wait, Wat? Resurrect a party member as a skeleton. Party members come back as skeletons if you don't resurrect them in the same chapter in which they died. Their stats are unaffected by this.
Daily Bread Complete a Daily Play. Daily Plays are a single chapter, so they don't take long to complete. The challenge is climbing the leaderboards.
Not The End Reach Level 50 in Endless Mode. Just keep on trying, this is one that you have to grind for.
Treachery Betray the Spymaster when the opportunity arises. This is how you unlock the route to the true ending.
True Calling Defeat The Eleven. After betraying the Spymaster, you'll have to take down his organization.

Tips

Su-po-po Use a Support Attack. If an ally is adjacent to your target after your attack resolves, they'll perform a Support Attack.
Triple Threat Defeat an enemy by having all three party members attack at once via Support Attacks. If the enemy dies before all three characters have attacked, it doesn't count for the Achievement.
Fancy Fencing Defeat 100 enemies with one-handed weapons. Fists don't count, you have to actually have a weapon equipped.
Stick 'Em Up Defeat 100 enemies with bows. Rogues and Rangers can use bows.
Pollsters Defeat 100 enemies with long weapons. Long weapons like spears don't usually appear at the Blacksmith until at least Chapter 2.
Skelet-owned Defeat 100 skeletons. Skeletons start appearing from Chapter 3 onwards in Story Mode, but they also appear in Daily Plays.
Human Trebuchet Throw 100 enemies off the board. You can only toss enemies out through open windows, doorways, stairs, etc. Be warned, they can do the same to you!
#NoElves Have 100 enemies killed by other enemies. The easiest way to set up enemies to hit each other is to get out of the way of ranged attacks, or dodge them.
Satisfaction Knock two enemies off the board at the same time. Try using Separation on the Stairwell map; with the right positioning, you can knock one enemy out the window and another down the stairs.
Cursed Cards Have three or more Curses in your hand at the same time. Warlocks excel at giving themselves Curses in exchange for extra benefits.
Big Hitter Deal 100 damage with a single attack. Use Steel Self to double your damage, along with Archetype multipliers and the Vulnerable debuff to do as much damage as possible.
Just A Little Guy Defeat a Giant (any enemy that takes up more than one tile). The Golem at the end of Chapter 2 is the first Giant you'll encounter in Story Mode.
Screaming Match Get a banshee to kill another banshee The boss fight against the King in Chapter 4 has two banshees; pit them against one another.

These Achievements require you to play through the game in a specific way, sometimes multiple times over.

Tips

A World Of Possibility Unlock every Archetype. Archetypes are unlocked with profile levels. Gain XP by completing runs, even if you are defeated.
Decks For Days Unlock every Card. Cards are also unlocked via XP, so just keep playing the game.
Old School Purist Complete Story Mode without buying any items. Brawlers are best for this, as they don't use anything but Consumables, which you can get as loot instead of buying.
Dragon Bedding Have 1,000 Gold at once in Story Mode. Take every dialogue option that offers Gold and don't spend any; try to complete every secondary objective in combat that you can.
Lone Wolf Complete Story Mode with a single character. Don't hire any party members at the Tavern, or accept any volunteers, for the entire run.
Class Act Beat Story Mode with all eight starting Archetypes. You're probably better at some classes than others; if you need to turn down the difficulty for classes that just don't click for you, that's fine.
