mysql-checkbox关联问题，求大家帮帮忙

我想高出图片这样子的页面系统，通过mysql数据库得到的信息显示在JSP页面上，在
通过选中checkbox，判断是否通过审核，将checkbox的值传到数据库中，可是我不知道怎么将数据库得到的名字与checkbox的值关联起来，然后传给数据库，我使用的是
servlet+JSP+mysql搞得。
求解答，或者有更好地方法，请给点建议，如果有例子的话更好，因为我是刚接触这些。谢谢！！！
