Aiko's Fate and its Consequences:

Paying Aiko results in his death from the venom, yielding 1,000 Skeyt (a 500 Skeyt profit) and his pelt. Refusal to pay still leads to his death, but without additional loot. Destroy nearby barrels to breach a wall and exit the camp with the package.
