In Kingdom Come: Deliverance 2's "Whom the Bell Tolls" quest, you'll be stripped of your gear at Trosky Castle and tasked with finding Thomas. This guide helps you navigate the castle, reach Thomas, and cure his illness.
Finding Captain Thomas:
After delivering sacks to the charcoal shed, speak with the blacksmith across the way. He'll request you craft a horseshoe (optional) granting you access to his workshop. You can also pilfer a lockpick from the shelf—useful later.
From the blacksmith's table, ascend the stairs to the right (the left door is a dead end). This area is restricted, so proceed cautiously. Climb the stairs, open the green door on your left, and follow the hallway to another staircase. A guard might be present; proceed with caution.
Another guard blocks the next staircase. Stealthily throw a rock to lure him away before ascending. Continue to the upper room where you'll find Thomas and his sister, Adela. She'll ask for your help in curing Thomas.
Curing Captain Thomas:
To cure Thomas, you need a specific medicine. Return to the blacksmith's area, and locate a door beneath the stairs. Inside, grab some trout.
Head back upstairs; a hungry guard will request food. Offer him the dried trout to gain passage. Continue up the stairs to a stone hallway, avoiding a patrolling guard. Enter the hallway with two doors, and choose the right door.
Inside, use your lockpick to open the chest (easy lock) containing Feverfew, Elderberry Leaves, and Ginger. A nearby book reveals you need a fever tonic.
At the alchemist's bench:
Return to Thomas and administer the tonic; a cutscene will trigger, completing this part of the quest.
